I used to play on normal, and then Ninja Gaiden series got me started playing on harder difficulties. Then I got hooked onto trophies so now I always try to beat hardest difficulty to unlock the trophies.
But then one day, I learned how to cheat, and I was like why did I put this much effort into playing against the CPU for a trophy when I should be going online and play with others?
Now I just beat the game on normal, use alternate means to get hard trophies, and then go online to play with others.

Normal, it's the perfect balance, once I get good enough I might play it again on hard.
Normal, except maybe racing games 'cause I don't need to worry much there. With other games I'd only start on Hard if I could later turn it down to Normal; if I don't know what to expect I'd just be frustrated if I'd get stuck somewhere ^^
I mostly play on normal. The only other time I change the difficulty to easy is to get a better handling for the game or maybe unlock an achievement or trophy. The only time I will play on hard is for an achievement/trophy or just because I really enjoyed the game.
Depends on the game. If there are RPG elements I will definitively play on normal, I hate grinding. Other than that if there's more than the usual three difficulty settings I go for hard. Usually in FPS, action and stealth games.
